SC construction stormwater forms: links to the agencies' own versions, plus free SWPPP HQ templates
Find links to SCDES and local stormwater program forms for South Carolina construction sites, on the agencies' own websites.
SCDHEC (DHEC) was split in two on July 1, 2024. Its environmental programs, including stormwater, moved to the new SC Department of Environmental Services (SCDES). Source: SCDES
The forms in sections 1 to 4 are the agencies' own. Each button there opens the agency's file, and the current version is always on the agency's site. Section 6 has free SWPPP HQ templates, which are not official forms.
1. Official SCDES forms you download and keep
These are filled out on paper (or in a PDF reader) and kept in the On-Site SWPPP. DES 0437 says contractor certifications "do not require Department approval"; each contractor gives the signed form to the owner/operator. Pre-construction conference attendance "must also be documented and maintained within the On-Site SWPPP (OS-SWPPP)" (DES 0436).
Pre-Construction Conference Certification Form (DES 0436)
A record that the pre-construction conference was held, where, who ran it, and who attended. Keep it in the On-Site SWPPP. (CGP 4.1, p.51)
Download PDF (525 KB)Contractor Certification Form (DES 0437)
Signed by each contractor who will work on the permitted site. Keep it in the On-Site SWPPP. A utility on an Annual Blanket NOI can use that coverage instead.
Download PDF (459 KB)SCDES SWPPP Template (C-SWPPP and On-Site SWPPP), including the official logs
Includes SCDES's own blank logs: SWPPP Inspection Log (p.32), SWPPP Rainfall Records (pp.36–37), Pre-Construction Conference Attendance Log (p.39), Contractor & Sub-Contractor Log (p.41), SWPPP Modification Log (p.43), and SWPPP Soil Stabilization Log (p.45). Those log pages can be printed on their own.
Download PDF (683 KB)Plan Review Checklist for Design Professionals
The SWPPP plan review checklist SWPPP preparers use. (SCDES's All Forms page still links an older version written for the 2012 permit. The version linked here is dated March 2026.)
Download PDF (1.5 MB)Permanent Stormwater System Maintenance and Responsibility Agreement
Required when the project has permanent stormwater BMPs (ponds and similar). (CGP 4.3.B–C, p.56)
Download PDF (246 KB)ePermitting Certifier Agreement
The permit says permittees "must sign and mail a wet-ink copy of the Certifier Agreement" to be able to sign applications electronically (CGP 2.5, p.20). Unconfirmed, verify with SCDES: whether online identity verification is also accepted (CGP 2.3, p.14).
Download PDF (766 KB)Expedited Review Agreement Form (DES 2618)
Only needed for projects in SCDES's expedited review process.
Download PDF (456 KB)The permit: NPDES General Permit SCR100000 (2026)
Keep a copy (or a link to it) with the On-Site SWPPP.

2. Forms filed online only, in SCDES ePermitting
SCDES says that after May 5, 2025, "all permit applications must be submitted exclusively through ePermitting." SCDES lists these forms as "Apply Online" and doesn't publish downloadable PDFs of them, so there is no download button here. Some MS4s post their own NOI forms for local review (section 4); check with your MS4.
Notice of Intent (NOI), primary permittee (D-2617)
Requests coverage under SCR100000. Contents: CGP 2.3.1, pp.14–15. Where to submit: CGP 2.5, p.20.
Open SCDES ePermittingNotice of Termination (NOT) (D-2610)
File it within 30 days of meeting a termination condition (CGP 5.1.B, p.58). Required contents: CGP 5.2.C, p.59. Inside an MS4, include the MS4's completion letter.
Open SCDES ePermittingTransfer of Ownership (D-0434, filed with an NOI)
For a new owner or operator taking over a permitted site. (CGP 2.4.3, p.17)
Open SCDES ePermittingMajor Modification (D-0435, filed with an NOI)
Open SCDES ePermittingIndividual Lot NOI, secondary permittee (D-0432) and Annual Blanket NOI, utilities (D-0433)
South Carolina doesn't use a separate "co-permittee" form. Builders on individual lots file as secondary permittees, utilities use the Annual Blanket NOI, and contractors sign DES 0437 (above). (CGP 2.2–2.3.2, pp.11–15)
Open SCDES ePermittingSites under 1 acre: Notification forms D-2628 (non-coastal) and D-0451 (coastal counties)
For sites disturbing less than 1 acre that are not part of a larger common plan. SCDES lists both as "Apply Online".

- Names and titles of the inspectors, and their qualifications if not given in an earlier report (unless they change)
- All discharge points
- Weather information and a description of any discharges at the time of the inspection
- Total rainfall since the last inspection
- Locations of discharges of sediment or other pollutants from the site
- Locations of BMPs that need maintenance
- Locations of BMPs that failed to operate as designed or proved inadequate
- Locations where additional BMPs are needed

- Site name, operator name, and permit number
- Verification that all BMPs in the OS-SWPPP are installed and operating as designed
- Whether the construction sequence is being followed
- Status of corrective actions from the previous inspection, with the date each was addressed
- Items carried over from previous reports that were not addressed
4. Local (MS4) forms
Large MS4s run their own review and inspection programs. Large MS4s may require their own forms in their area; check with your MS4. We checked these six on Oct 20, 2026:
Anderson County: NPDES Storm Water Construction Inspection Report (Appendix H)
Anderson County's Design Manual (June 2026) says that for each required site inspection "an inspection report, provided in Appendix H, must be completed." Check with Anderson County Stormwater for how it applies to a project.
Download PDF (24 KB) County NOT: PDF (45 KB) County NOI application 2026: PDF (759 KB) Notification of Start: PDF (44 KB)Charleston County: Stormwater Inspection Report (Appendix G)
A filled-in sample of the county inspector's own report (2018). It is not a blank form for the permittee's weekly inspections. The county's stormwater applications are on its stormwater page.
View sample: PDF (143 KB)City of Columbia: Land Disturbance Application / NOI
The city doesn't publish a construction inspection form.
Download PDF (236 KB)Greenville County
The county requires an inspection report for each inspection, kept on site, but doesn't publish a form for it (per the county's Land Development Permitting/Forms page, as recorded in our Oct 20, 2026 check). Its Permitting/Forms page has the county's other forms, including a copy of the SCDES NOI (DES 2617, 01/2025).
Open Greenville County forms pageSpartanburg County and Richland County
Unconfirmed, verify with Spartanburg County or Richland County: whether the county has its own construction inspection report form. Their websites blocked our check on Oct 20, 2026.
5. Unofficial blank forms (not SCDES forms)
These are not SCDES or MS4 forms.
- Clemson CEPSCI program: CEPSCI Compliance Inspection Report (DOCX, 221 KB). Clemson labels it "Modified from SCDES 0496", Version 1, 2025. Clemson's resources page also hosts Word copies of the SCDES template logs and the older DHEC 0496 (03/2013) inspection report, which SCDES itself no longer publishes.
6. Free SWPPP HQ templates (not official forms)
Blank forms from SWPPP HQ, for places where we found no official form. Fill them in on screen or print them. Each field is marked Required, Required if applicable, or Optional, and shows where it comes from: a permit or regulation section, your maintenance agreement, or "not a permit item".
Weekly SWPPP inspection report
For construction sites in South Carolina, except Anderson County. In Anderson County, use the county's own Appendix H inspection report (PDF, 24 KB). Spartanburg County and Richland County: we couldn't confirm whether the county has its own form, so check with the county.
Free template from SWPPP HQ, an independent site. Not an official SCDES, county, or MS4 form. Check your permit, SWPPP, and local MS4 for what's required.
Fillable PDF (38 KB) Word .docx (39 KB) Excel .xlsx (8 KB)No one-page version: the 17 required items need more room than one page.
Corrective-action / BMP maintenance log
Supplements inspection report items 4.2.F.X, XIV and XV. It doesn't replace them.
Free template from SWPPP HQ, an independent site. Not an official SCDES, county, or MS4 form. Check your permit, SWPPP, and local MS4 for what's required.

- Greenville County (dry detention ponds): Dry Detention Basin Operation Inspection and Maintenance Record (PDF, 109 KB)
- Lexington County: Stormwater Pond Annual Inspection Checklist for BMP Owners (PDF, 513 KB)
- Horry County: Sample Self Inspection Checklist (PDF, 20 KB)
Other counties and cities: not offered yet.
Free template from SWPPP HQ, an independent site. Not an official SCDES, county, or city form. Check your recorded maintenance agreement, your county or city stormwater ordinance, and your approved maintenance plan for what's required. If your county or city has its own form, use theirs.
Fillable PDF (22 KB) Word .docx (38 KB) Excel .xlsx (7 KB)One-page version only: every field is optional and the list fits on one page.
Not offered here: a rain gauge log. Your SWPPP may use SCDES's "SWPPP Rainfall Records" (SWPPP Template App. F, pp.36–37) in the SCDES SWPPP Template (PDF, 683 KB), or another log. Use the one your SWPPP calls for. The same SCDES template has blank modification, soil stabilization, pre-construction conference attendance, and contractor logs.
About this page
Last checked Oct 20, 2026.
Each listing shows the publisher, the file type and size, and the version printed on the form, where there is one. Sizes are the file size the agency's server reported on Oct 20, 2026. Citations like "CGP 4.2.F, p.54" mean section 4.2.F of permit SCR100000, on page 54 of the PDF.
Disclaimers
SWPPP HQ is an independent guide, not affiliated with SCDES or any county, city, or MS4. The forms in sections 1 to 4 are published by the South Carolina Department of Environmental Services (SCDES, formerly SC DHEC) and by local MS4 programs. They are the agencies' forms, not ours. The current version is always the one on the agency's own website. Every download button in sections 1 to 4 opens the agency's own file. We don't host copies of agency forms. This is general information, not legal or engineering advice. Forms and links can change, so check the agency's page before you file. If your site is inside a regulated MS4 (for example Greenville County or Anderson County), the MS4 may require its own forms as well.
